At NYU Langone Health Home Care, we provide a range of specialized services and support that allow you to continue your care in the comfort and safety of your home while maintaining close communication with your doctor.

Our providers of skilled medical, nursing, rehabilitation, and medical social work services are committed to treating each patient with dignity, respect, and culturally sensitive care.

Licensed by the New York State Department of Health and accredited by The Joint Commission, we have been recognized annually for more than 35 years by DecisionHealth with the prestigious HomeCare Elite^® national award for excellence in home health services.

Our services are available to adults in the five boroughs of New York City, as well as Nassau and Suffolk counties on Long Island. Home health services can be arranged through a care management nurse as part of your discharge plan from any NYU Langone hospital or health-related facility. If you are at home, your physician can call us at 516-663-8000 to arrange for your in-home care. We accept Medicare, Medicaid, and private health insurance plans.

Services We Provide

Our registered nurses and therapists consult with your doctor to develop a home healthcare plan tailored to your needs, and coordinate the following services:

• home health aides
• intravenous therapy services including antibiotics, chemotherapy, hydration, and total parenteral nutrition
• medical equipment and supplies
• physical, occupational, and speech therapy
• skilled nursing
• medical social work
• telemonitoring

Our Specialized Care Programs

Our specialized care programs are designed to ensure a smooth and safe transition to in-home care. We work closely with you to help you understand and manage your healthcare needs, including medication, nutrition, and device care. We offer a range of in-home programs for a variety of healthcare needs, including the following conditions and specialty services:
